MCPN Health & Wellness Center at APDC is a full-service, family practice primary care clinic specializing in culturally and linguistically competent care for refugees and immigrants

  • Services include adult immunizations, chronic disease management, prevention and health education, physicals, women’s wellness (Pap smear, CBE, referral to mammography), well child checks, wound care, referrals to specialty care, and much more!

  • Medically certified, bilingual patient navigators provide additional support to all patients seen in the primary care clinic, assisting with navigating the healthcare system, managing prescription medications, adopting healthier lifestyle habits and achieving the highest level of health and wellness possible.

  • MCPN collaborates closely with APDC providers and staff to provide team-based, whole person care that draws on expertise from across disciplines and is highly responsive to the full spectrum of a patient’s needs.

  • Hours of operation: Monday-Thursday, 8:00AM-4:30PM. Appointments can be made in person (MCPN front desk at APDC) or by phone 303.360.6276.
